22VAC40-665 Subsidy Program Vendor Requirements for Family Day Homes

This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
Preliminary data was obtained from the vendor on 1/11/21.
-An annual subsidy inspection was initiated on 1/12/21, at which time the vendor was contacted by telephone to initiate the inspection, and concluded on 1/13/21. The inspector emailed the vendor a list of items required to complete the inspection.
The vendor reported there were 4 children in care and no children living in the home under the age of 12 present with the vendor. The inspector reviewed 2 children?s records and 2 household member records submitted by the vendor and health and safety regulations.
-Information gathered during the inspection determined an area of non-compliance with applicable regulations or law, and a violation were documented on the violation notice issued to the vendor.

Standard #: 22VAC40-665-170-B Description: The vendor failed to get some of the required paperwork to make the child's information complete.
Evidence: Based on the review of two children's files , one child , child #2 is missing an emergency contact that is different from the parents.Plan of Correction: I will ask the parents to add an emergency contact that is different from them.
