Comments:
This annual subsidy inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated and completed on 2/4/2021. The vendor was contacted by telephone to initiate the inspection. There were 3 children present at the time of the inspection directly supervised by the vendor.The inspector reviewed 3 children's records, household member information and discussed requirements to ensure vendor was in compliance with health and safety regulations.

The inspector reviewed with the vendor COVID 19 Essential Guidance for Child care Programs.

The information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Description: Based upon information reviewed required child information was found to be incomplete.
Evidence:
Child A, B and C were missing parent work address and proof of identity.

Plan of Correction: I will get forms and complete.
