This inspection was conducted by staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time however, a portion of the inspection included a virtual component which allowed the inspector to observe the daycare areas of the home.
An unannounced subsidy inspection was conducted at 10:00am. The vendor was contacted by phone to initiate the inspection, and provided a list of items required to complete the inspection. There was 1 child present = 1 point with the vendor. The daycare was sufficiently supplied with toys and equipment for children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliances with applicable standards or laws reviewed. Standard #: 22VAC40-665-170-B Description: Based on records reviewed, it was determined that 1 of 1 children's records did not include required information. Evidence: Child #10 - There was no information for the 2nd parent, and information for one parent was written in both sections of the enrollment paperwork. Plan of Correction: The vendor will discuss with parent, and correct the enrollment paperwork. If there is not a 2nd parent to be listed, "N/A" should be written.
Standard #: 22VAC40-665-210-A Description: Based on records reviewed, it was determined that 1 of 1 child records did include documentation of current immunizations. Evidence: Child #10 - The immunizations were dated September 2011 when the child was 6 months old, and were not those of at least 2 years old. The child is currently 9 years old. Plan of Correction: The vendor will request current immunizations from parent.
Standard #: 22VAC40-665-230-H Description: Based on records reviewed, it was determined that the vendor did not complete a training course by the due date of 3/1/20. Evidence: The training certificate was dated as completed on 2/12/21. Plan of Correction: Completed.
