Comments:
An unannounced renewal inspection was conducted on July 5, 2018, between the hours of approximately 12:45 p.m. and 4:00 p.m., with follow-up conducted on July 10, 2018 and July 11, 2018. Sixty-eight (68) children were enrolled and forty-nine (49) children were present, on July 5, 2018. Staff included eight to eleven counselors, two site directors and two CITs (counselors-in-training). Five children's records, three children's medications, three topical ointments, accompanying authorization forms and medication administration logs were reviewed. Program observation was completed, including the observation of: a field trip at Gypsy Hill Park; restroom and handwashing procedures; transportation via school bus; lunch (nachos with cheese and salsa, sunbutter sandwiches, cucumbers, hummus, watermelon and milk); outdoor play; gym time; transitions, afternoon snack (poptarts) and departure of children. Review of five staff records, five children's records and background clearances/references for board officers/agents was completed at the program's administrative office on July 11, 2018, between the hours of approximately 2:05 p.m. and 5:30 p.m. Inspection findings were discussed with staff at both locations. Previously cited standards violations (inspection date - 7/05/2017)were reviewed for compliance. No repeat violations were cited. Refer to the accompanying violation notice for exceptions to compliance. The time and assistance of staff members was appreciated during the renewal process. Standard #: 22VAC40-185-70-A
Description: Based on review of five staff records and discussion with staff on July 11, 2018, the program failed to obtain verification of age and documentation of one reference check, for one staff member. Evidence: 1. Review of staff #5's record, on this date, determined that verification of age was not included in the record. 2. Review of staff #5's record revealed that there was documentation of only one reference check on file. 3. Staff #6 confirmed the lack of documentation in the staff record.

Plan of Correction: Reference and age verification will be obtained before training and staff will not be allowed to work, if not received.

Standard #: 22VAC40-185-540-E
Description: Based on observation of emergency supplies and discussion with staff on July 5, 2018 and July 10, 2018, the program failed to have a working battery operated radio on site. Evidence: 1. Review of emergency supplies on July 5, 2018 revealed that there was not a battery-operated radio on site. 2. Review of emergency supplies on July 10, 2018 found that a battery-operated radio was on site but there were no available batteries. 3. Staff confirmed the lack of a battery-operated radio and the lack of available batteries.

Plan of Correction: Batteries were purchased for the radio.

Standard #: 22VAC40-185-560-F
Description: Based on observation of afternoon snack and discussion with staff on July 5, 2018, the program failed to follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: 1. Observation of afternoon snack by the licensing inspector (L.I.), on this date, determined that only one food component was served to the children (poptarts). Two food components were required, by USDA, to be served for snacks. 2. Discussion with staff, on this date, determined that only one food component was served for snack each afternoon during summer camp.

Plan of Correction: Milk was purchased for the site and will be offered with the afternoon snack.
