Thank you for your time and assistance with the unannounced monitoring inspection conducted on July 23, 2018. The on-site inspection was held from 11:45 a.m. to 2:35 p.m. On this date, there were 22 children in the care of seven staff members with extra staff members available to assist if needed. There were a total of 22 children enrolled in the camp this week. The exceptions to compliance were noted on the violation notice and the risk ratings were discussed. There were eight staff records and five children's records reviewed. There were no medications on-site or being administered. Four staff and four children were interviewed. The theme for camp this week was On The Farm. Some of the items observed and reviewed were required posted information, evacuation and shelter-in-place drill documentation, annual health and fire inspections, sunscreen authorizations, first aid kit, lunch, transitions, and indoor games and activities. Standard #: 22VAC40-185-160-C Description: Based on review of eight staff records and verification from the director, one staff member had not obtained a tuberculosis (TB) screening within the last two years. Evidence: 1. The record of Staff E did not contain results of an updated TB test/screening. Staff E confirmed that the date of her last TB test was 06/03/2015. Plan of Correction: The staff member will obtain an updated TB screening as soon as possible.
Standard #: 63.2-1720.1-B-3 Description: Based on review of eight staff records and verification from the director, the center failed to obtain results of a search of the central child abuse and neglect registry for the two states of which two staff members had resided in the last five years. Evidence: 1. The record for Staff E showed she had resided in a state outside of Virginia within the last five years. There was no documentation that a search of the registry had been obtained from that state. Date of hire for Staff E was 07/21/2014. 2. The record for Staff G showed she had resided in a state outside of Virginia within the last five years. There was no documentation that a search of the registry had been obtained from that state. Date of hire for Staff G was 06/04/2018. Plan of Correction: The director will ensure the requests for these searches will be completed immediately. She will provide documentation to the licensing inspector that the requests have been sent within ten days. Once, submitted, if they are not completed and received within 45 days, she will follow-up with each state.
