Technical Assistance:
New background check requirements and New Sworn Disclosure or Affirmation form were discussed. The number to OBI was provided to the center director.

Comments:
An unannounced renewal inspection was conducted today from 9:40am-11:50am. There were 18 children (3yrs-8yrs.) directly supervised by 2 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, using the gym area to engage in large motor skills activities, eating snack and doing crafts. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Description: Based on record review, children's files were incomplete.

Evidence:
Child#1's record is missing the address for each emergency contact. child#4's record is missing the father's employer name, and a work telephone number. Child#5's record is missing the father's employer name, a work telephone number and a complete address for 1 of 2 required emergency contacts.

Plan of Correction: Director will request the information from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on record review, areas and equipment of the center were not maintained in a safe and operable condition.

Evidence:
The Licensing Inspector observed a small hole in the wall, In the gym area by the steps that lead to the back room.

Plan of Correction: We'll have the hole patched and fixed.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of four staff records, the center did not have documentation of a central registry report for a staff member within 30 days of employment.

Evidence:
Staff#1's file does not contain a central registry report, the staff member was hired on 4/1/2017

Plan of Correction: Obtaining documents from the staff's prior gym location.

Description: Based on review of four staff records, the center did not have an original criminal history record report within 30 days of employment of a staff member. Evidence: Staff#1's record does not contain a criminal history report, the staff member was hired on 4/1/2017.

Plan of Correction: Obtaining documents from the staff's prior gym location.
