Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, June 6, 2019. The on-site inspection was from 1:20 p.m. to 4:10 p.m. On this date, there were 44 children present at the summer camp and in the care of six staff members. There was one additional staff member available to assist if needed. Nine staff records were reviewed at the Staunton-Augusta YMCA administration office from 11:30 a.m. to 1:05 p.m. on this date. There were 48 children enrolled in camp this week. Five children's records were reviewed. Three small groups of children were observed. Six staff members and three children were interviewed. The medication and medication authorization were reviewed for two children. The exceptions to compliance were noted on the violation notice and the risk ratings assigned to the violations were noted on the supplemental page. Some items reviewed/observed: outdoor play on the playground, transitions, large motor play in the gym, art activities, snack afternoon snack, required posted information, over-the-counter skin products and parental authorizations, injury reports and dismissal procedures. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of nine staff records and verification from staff, the center failed to obtain a second reference for one staff member. Evidence: 1. The record for Staff Member #2 did not contain a second reference as to his character, reputation and competency. 2. Staff Member A verified that there was only one reference in the file for Staff Member #2.

Plan of Correction: The administration will obtain a second reference as soon as possible.

Standard #: 22VAC40-185-500-A
Description: Based on observation and confirmation from staff, the center failed to have the children wash their hands after eating snack. Evidence" 1. All of the children finished their snack and staff directed them to the next activity without having the children wash their hands with soap and running water or disposable wipes. 2. Staff Member #9 confirmed that she forgot to have the children wash their hands after snack.

Plan of Correction: Starting tomorrow the staff will have all children wash their hands after eating every meal or snack.

Standard #: 22VAC40-185-560-K
Description: Based on observation, review of cleaning products and discussion with staff, the center failed to sanitize the tables used for eating. Evidence: 1. The staff did not sanitize the tables used for meals and snacks before or after afternoon snack. They cleaned the tables with a commercial cleaner but did not have any type of disinfecting product on-site to sanitize the tables. 2. Staff Member A stated she thought the cleaner being used could also be used as a sanitizer.

Plan of Correction: Staff Member A will get a sanitizing solution to the center and retrain staff on how to sanitize the tables before and after eating and clean the tables at least once a day.
