Comments:
An unannounced monitoring inspection was conducted on 07/03/2019 and 07/08/2019. There were 113 children, ages 4 years-12 years, and 19 staff members present during the on-site inspection. The inspector reviewed 10 children's records and five medications during the 07/03/2019 inspection and 10 staff records during the record review inspection at the corporate office on 07/08/2019. The children were observed in the following activities: arriving to the program, having breakfast, participating in arts and crafts, having free choice play, in organized large motor play in the gym and going outside to play. The inspector reviewed the required the parent handbook and the emergency supplies during the inspection. The inspector discussed the following with the staff: medications, ratios, sunscreen, staffing and background check requirements. The inspector arrived for the 07/03/2019 on-site inspection at 9:00 a.m. and departed at 11:15 a.m. and arrived for the 07/08/2019 record review inspection at 9:00 a.m. and departed at 12:20 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-50-A
Description: Based on observations and discussion with the staff, the facility failed to ensure that children's records shall be treated confidentially. Evidence: There were copies of 2 children's enrollment forms on the top of the sign-in sheet clipboards at the parent sign-in/information table. These 2 children's enrollment forms were accessible to anyone that signed a child in or out of the program. These 2 enrollment forms were not treated confidentially.

Plan of Correction: The director will ensure that all paperwork will be given directly to teachers and not placed on the clipboard.

Standard #: 22VAC40-185-70-A
Description: Based on record review and discussion, the facility failed to ensure that the staff records shall contain all of the elements as required by the standards. Evidence: 1. The record for Staff #2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment; the documented date of hire is 05/23/2019. 2. The record for Staff #4 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment; the documented date of hire is 05/14/2019.

Plan of Correction: The director will ensure that reference checks are done prior to hire.

Standard #: 22VAC40-185-280-D
Description: Based on observations and discussion with the staff, the facility failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles. Evidence: There were 4 containers of Citrus Clear Deodorant and 2 containers of disinfectant wipes stored in the same drawer as the single-service items of the first aid kit (band-aids, gloves, gauze, etc. ).

Plan of Correction: The first-aid items were moved during the inspection.

Standard #: 22VAC40-185-340-F
Description: Based on observations and discussion with the staff, the facility did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: Child #1 was observed alone in the Main Hallway in front of Classroom 301. The child was observed walking alone down the Main Hallway, to the stairwell, down the stairs to the lower level, around a corner, and into the cafeteria to where the staff and children were having breakfast. The child was out of the sight and sound supervision of staff during this time. Child #1 was identified to be 6 years old.

Plan of Correction: The director met with the staff on 07/08/2019 and discussed: staff persons in the front/back of lines, head counts and taking their time during transition times.

Standard #: 22VAC40-185-350-E-4
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that for children from three year of age to the age of eligibility to attend public school, five years by September 30, a one staff member for every 10 children ratio is met. Evidence: There were 85 children and 5 staff members observed in the gym prior to the arrival of the 8:30 a.m. staff arriving. Child #6 was identified as the youngest child in the gym at that time. Child #6 was identified to be 4 years old. The ratio for 4 year old children is 1 staff member for every 10 children.

Plan of Correction: The kindergarten children are being separated from the other groups during the 6:00 a.m.-8:00 a.m. to meet ratio.

Standard #: 22VAC40-185-510-G
Description: Based on record review and discussion with the staff, the facility failed to ensure that the center may administer prescription medication that would normally be administered by a parent or guardian to a child provided that the center has obtained written authorization from a parent or guardian. Evidence: There was written documentation that Medication D had been administered on 06/06/2019. There was not written authorization from a parent or guardian to administer Medication D available for review during the 07/03/2019 inspection.

Plan of Correction: The written medication permission form was obtain during the inspection.

Standard #: 22VAC40-191-60-B
Description: Based on record review and discussion with the staff, the facility failed to ensure that persons must not be employed until the agency has the persons completed sworn statement or affirmation. Evidence: The record for Staff #4 has a documented hire date of 05/14/2019; the date on the sworn statement was 05/15/2019.

Plan of Correction: The sworn statement will be signed on the date of hire.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and discussion with the staff, the facility failed to obtain a search of the central registry findings within 30 days of hire. Evidence: 1. The Record for Staff #1 did not contain the results of a search of the central registry; the documented date of hire in the record for Staff #1 is 05/15/2019. 2. The Record for Staff #2 did not contain the results of a search of the central registry; the documented date of hire in the record for Staff #2 is 05/23/2019. 3. The Record for Staff #3 did not contain the results of a search of the central registry; the documented date of hire in the record for Staff #3 is 05/22/2019. 4. The Record for Staff #4 did not contain the results of a search of the central registry; the documented date of hire in the record for Staff #4 is 05/14/2019. 5. The Record for Staff #5 contained a search of the central registry dated 04/25/2019; the documented date of hire in the record for Staff #5 is 03/16/2019. 6. The Record for Staff #9 did not contain the results of a search of the central registry; the documented date of hire in the record for Staff #6 is 05/06/2019.

Plan of Correction: The director will ensure that they are done within 30 days of hire.
