Comments:
A Subsidy Health and Safety inspection was conducted on 07/03/2019 to review supplemental health and safety requirements. There were 113 children, ages 4 years-12 years, and 19 staff members present during the inspection. The inspector reviewed 10 children's records and five medications during the inspection; the staff records will be reviewed on a later date at the corporate office. The children were observed in the following activities: arriving to the program, having breakfast, participating in arts and crafts, in free choice activities, in organized large motor play in the gym and going outside to play. The inspector reviewed the parent handbook and the emergency supplies during the inspection. The inspector discussed the following with the staff: medications, ratios, sunscreen and staffing. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and can be found on the Violation Notice. Description: Based on record review and discussion with the staff, the facility failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child #2 contained documentation of a diagnosed food allergy. There was not a written allergy care plan on site for Child #2.

Plan of Correction: The parent was contacted during the inspection and stated the allergy no longer exists. The staff asked the parent to update the records.
