Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:15pm. There were 117 children in the supervision of 18 staff. A sample size of 10 children's records, 5 staff records, 2 minor injury reports, 7 emergency medications, the first aid kit, emergency materials, evacuation drills, and the physical plant were reviewed. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, 2 out of 5 staff records were incomplete. Evidence: 1. Staff #3 was missing documentation of 2 out of 2 required references (DOH: 6/6/2019). 2. Staff #5 was missing documentation of 2 out of 2 required references (DOH: 6/6/2019).

Plan of Correction: We will get the references for these employees as soon as possible.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medications, the Provider did not ensure that prescription medication was dispensed from a pharmacy and maintained in the original, labeled container. Evidence: 1. There was no prescription label on an inhaler of albuterol with the name of child #1 written on the bag it was inside of. 2. There was no prescription label on the Epi-Pens (4) with the name of child #1 written on the the bag they were inside of.

Plan of Correction: We will ask the parents for the prescription labels and boxes.
