Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:30 AM and departed at 1:40 PM. Ninety four children were in care. Activities, reading classes, return from swimming and lunch service were observed. Medication administration and a sampling of injury records were reviewed. A sample size of nine children's records and nine staff records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. There was a container of antibacterial wipes and a pump container of hand sanitizer, both labeled "warning, keep out of reach of children", on a shelf by the teacher's desk in the fifth grade classroom. 2. In the kindergarten classroom, there was Gold Bond eczema lotion and a tube of cocoa butter, both labeled "warning, keep out of reach of children" in a drawer of the teacher's desk that did not have a locking method. 3. The lock on the storage area under the hand washing sink in the art room where numerous hazardous cleaning products are stored was open/not locked.

Plan of Correction: The facility responded with the following: All staff will be reminded that hazardous substances must be kept in a locked location using a safe locking method at all times.

Standard #: 22VAC40-185-510-G
Description: Based upon review of medications stored for potential emergency administration to children, the facility has not ensured that medication that would normally be administered by a parent or guardian would be administered by a staff member who meets the requirements in 22VAC40-185-240. Evidence: 1. There is diazepam stored for emergency should child 10 have a seizure. The diazepam is to be rectally administered. 2. The program director acknowledged that there are currently no staff who have the required medication administration of rectally administered medications training certification to be able to administer the medication. 3. There was no written documentation on the medication itself or the authorization forms that staff are not to administer the medication or any alternative action to take in an emergency.

Plan of Correction: The facility responded with the following: Written arrangements will be made with the parent regarding actions to take should the child have a seizure, ensuring that the parent is aware that no staff are trained to be able to administer the medication. If the medication is kept on site to be given to paramedics in an emergency, it will be clearly labeled to indicate that staff are not to administer the medication.
