Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 25, 2019. The on-site inspection was from 1:50 p.m. to 4:40 p.m. On this date, there were 73 children present at the summer camp and in the care of 12 staff members. There were three additional staff members available to assist if needed. Nine staff records were reviewed at the Staunton-Augusta YMCA administration office 0n 06/06/2019. There were 102 children enrolled in camp this week. Eight children's records were reviewed. All small groups of children were observed. Five staff members and five children were interviewed. The medication and medication authorization were reviewed for four children. The theme for the week was Shining Stars. The exceptions to compliance were noted on the violation notice and the risk ratings assigned to the violations were noted on the supplemental page. Some items reviewed/observed: outdoor play on the playground, transitions, eight outside play stations, large motor play in the gym, afternoon snack, required posted information, warm water temperature, documentation of evacuation, shelter-in-place, and lockdown drills, over-the-counter skin products and parental authorizations, injury reports and dismissal procedures. Standard #: 22VAC40-185-510-A
Description: Based on review of four medications and medication paperwork and verification with staff, the medication for one child did not contain parental authorization allowing the center to administer the medication. Evidence: 1. The medication paperwork for Child D contained parental authorization that expired on 06/13/2019. 2. Staff confirmed that the authorization was no longer valid.

Plan of Correction: The staff will take the medication and return it to the parent.

Standard #: 22VAC40-185-550-C
Description: Based on observation, a tour of the center, and confirmation from staff, the center failed to post an emergency evacuation and shelter-in-place map on each floor of the building and in a location that could be seen by staff and children. Evidence: 1. There was no emergency evacuation map or shelter-in-place map posted on either floor of the building. 2. The staff confirmed they had not posted the maps at the new location.

Plan of Correction: In the future, the center will have evacuation and shelter-in-place maps posted on every floor of the building.
