63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted today form 11:00am until 2:00 pm. There were 6 children present with 3 staff members. The children were observed doing floor exercised and using the air mat. The physical plant, children's records, staff records, emergency plans, emergency supplies, evacuation drill logs and attendance logs were inspected. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of 5 staff records, in 3 out of 5 records, the center failed to insure that staff members will obtain and submit the results of a TB test or screening within 21 days of hire or within 12 months prior to hire. Evidence: 1) Staff #1's TB test expired 6/5/19. 2)Staff #2 and #3's filed did not contain documentation of TB screening or test.

Plan of Correction: I will contact staff and ensure they have TB testing or screening.

Standard #: 22VAC40-185-60-A
Description: Based on a review of 3 children's records, in 1 out of 3 records, the center failed to maintain records for each child enrolled. Evidence: Child #2 did not have documentation for emergency pick up authorization.

Plan of Correction: I will contact parent and get the information for the child's record.

Standard #: 22VAC40-185-70-A
Description: Based on a review of 5 staff records, in 1 out of 5 records, the center failed to maintain staff records. Evidence: 1) Staff #2 and #3 did not have documentation of a second reference. 2) Staff #5 did not have documentation of 2 references.

Plan of Correction: I will contact references and document staff files.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances will be kept in a locked place using a safe locking method that prevents access by children. Evidence: The soap used in both bathroom's is labeled keep out of reach of children.

Plan of Correction: Removed during inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of 5 staff records, in 2 out of 5 records, the center failed to ensure that staff would obtain central registry findings within 30 days of employment. Evidence: Staff #2 and #3's files do not have documentation of central registry record checks.

Plan of Correction: Forms have been sent for CPR checks, but have not been returned. I will contact the agency to inquire.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of 5 staff records, in 1 out of 5 records, the center failed to ensure that staff submit to fingerprinting and provide personal descriptive information. Evidence: Staff #4 (date of hire 3/8/`6) did not have documentation of a fingerprint record check in the staff file.

Plan of Correction: I will get fingerprints taken.
