Comments:
An unannounced monitoring inspection was conducted from 10:20am-12:19pm. There were 97 children in the direct supervision of 16 staff members. A sample of 9 children's records, 5 staff records, emergency materials, and the physical plant were reviewed. The children were observed dancing, playing outdoor games, playing sharks and minnows in the gym, and swimming at the outdoor pool. Standard #: 22VAC40-185-70-A
Description: Based on a review of children's records, 3 out of 9 were incomplete. Evidence: 1. Child #5 was missing 2 out of 2 required addresses both emergency contacts. 2. Child #6 had incomplete addresses for 2 out of 2 required emergency contacts. 3. Child #9 had incomplete addresses for 2 out of 2 required emergency contacts.

Plan of Correction: We will ask the parents for these at pick-up time today.
