A announced monitoring inspection was conducted with the director. There were 27 children in care with a age range of five years to eleven years old. The younger children were using the outside pool and the older children were in the gym playing basketball. Six staff and children's records were reviewed. All required paper work was posted. Standard #: 22VAC40-185-60-A Description: Based on review of six children's files, not all had a separate file for each child enrolled. Evidence: Child A and B's were in the same file. Plan of Correction: Files will be separated and each child will have a file.
Standard #: 22VAC40-185-510-A Description: Based on review of the children's medication not all had a parents authorization from the parent to give the medication. Evidence: Child A did not have a parent's authorization to give a medication that was stored in the medication box. Plan of Correction: We will have the parents sign the authorization.
Standard #: 22VAC40-185-540-C Description: Based on review the first aid kit was not complete. Evidence: The first aid kit only had one triangular bandage. Plan of Correction: We will buy another triangular bandage.
