Discussed: -Updated version of the Sworn Statement - procedures for completing out of state central registry check for employees living outside of Virginia within the last 5 years ( Adam Walsh web site) -new requirement that all searches are to be repeated every 5 years: FBI fingerprint Central registry check ( child abuse and neglect) sworn statement

Comments:
An unannounced monitoring inspection was conducted today 11/1/2018 from 12:00 pm- 2:00 pm. Staff to child ratios and regular attendance were discussed during the inspection. On the day of inspection, administration was in a classroom to maintain ratios due to staff illness. Inspector observed children playing outside on the playground before dismissal. Director gave inspector a tour of all classrooms and staff reviewed policies and procedures regarding hand washing, potty routines and cleaning and sanitizing of classrooms. Training for daily health observations, First aid and CPR and mandated reporting was also discussed. A sample of staff records and written disclosures to parents were also reviewed. Standard #: 63.2(17)-1716-A-6
Description: Based upon observation and interview, required written disclosure of operations was not available to parents as required. Evidence: Public liability insurance could not be located in a way that was disclosed to parents in writing.

Plan of Correction: Program will decide to post required information where it used to be located in office or update handbook.

Standard #: 63.2(17)-1720.1-A
Description: Based upon records reviewed, required searches were not on record. Evidence: Staff A- did not have central registry check completed within the last 5 years of employment and did not have an updated sworn disclosure statement on file. Staff B- did not have central registry check completed within the last 5 years of employment and did not have an updated sworn disclosure statement on file. Staff C- did not have central registry check completed within the last 5 years of employment and did not have an updated sworn disclosure statement on file. Staff D - did not have central registry check completed within the last 5 years of employment and did not have an updated sworn disclosure statement on file.

Plan of Correction: Administration had all staff members complete updated sworn disclosure statements while inspector was on-site. Central registry checks were initiated while inspector was on site and will be completed and mailed within the next 10 days. ( schools are closed Nov. 5-6 ) date reflects additional days to complete.
