Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on August 5, 2020 and concluded on August 5, 2020. The director was contacted by video call to initiate the virtual inspection. There were five children present and two to four staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and a violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-550-M
Description: Based on a review of written records of children's minor injuries, and an interview with administrative staff, the program did not ensure that all required documentation was recorded on each day of all occurrences. Evidence: A "First Aid Log" was used to document three of four minor injuries treated. The minor injury on August 5, 2020 did not include the child's name. The minor injuries documented on July 29, 2020, July 31, 2020 and August 5, 2020 did not include circumstances of the injuries, names of all staff present, method, date and time of parent notification, actions to be taken to prevent future incidents, two signatures by staff or a parent signature with one staff signature.

Plan of Correction: The director will instruct the staff to complete the incident report form for injuries that occur. The director will review the requirements for documenting injuries as required by the licensing standards during the summer staff orientation. The incident report form will be updated to include a place to document the time and method of parent notification.
