The Licensing Inspector for Fresh Start Ministries (God?s Creative Learning Center) conducted an unannounced Monitor Inspection on Tuesday, March 5, 2019. Upon arrival at 12:40 pm there were a total of nineteen (19) children present under the direct care & supervision of four (4) caregivers. Five (5) children records & four (4) staff records were reviewed during today?s inspection. Children were observed during arrival/departure procedures, bathroom/hand washing, snacks/meal served, group activities & self-initiated activities. Licensing Inspector conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director/Administrator on March 5, 2019. Description: Based on record review, the Center failed to ensure each employee, volunteer or any other person who is expected to be alone with one or more children enrolled in the religious exempt child day center completes a sworn statement or affirmation before employment at the facility. EVIDENCE: Licensing Inspector reviewed 4 staff records. Record for staff #3 did not include a complete sworn statement/affirmation

Description: Based on inspection, the Center failed to provide required information to the parents of the children enrolled. EVIDENCE: Qualifications of personnel was not disclosed in writing to the parents of the children enrolled.

Description: Based on record review, the Center failed to ensure each staff person has been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision. EVIDENCE: Licensing Inspector reviewed 4 staff records. Record for staff #4 did not include documentation of a health exam.

Plan of Correction: Director will obtain a health examination within 2 weeks

Description: Based on inspection, the Center failed to describe in a required written statement to the parents/guardians of the children in the center and made available to the general public EVIDENCE: Written statements regarding the physical facilities, health requirements for the staff and public liability insurance were unavailable.

Plan of Correction: Will update parent handbook to include required information.

Description: Based on record review, the Center failed to ensure staff member's submit to fingerprint-based criminal history check determination letter. EVIDENCE: Licensing Inspector reviewed 4 staff records. 2 out of the 4 staff members did not complete/submit fingerprint-based criminal history prior to the 1st day of employment. Staff #2 date of hire is 1/18/2001; Staff #3 date of hire is 4/08/2002

Plan of Correction: Staff members will not work until fingerprint background checks are completed. Director will submit proof of correction the the Licensing Inspector within 10 days.
