This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 06/17/2021 and was conducted on 06/23/2021. The provider was contacted by telephone to initiate the inspection. There were 97 children and 18 staff present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 5 medications during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Standard #: 22VAC40-191-60-B Description: Based on record review and discussion with the staff, the facility failed to ensure that a person must not be employed until the agency has the person completed sworn statement or affirmation.
Evidence: The record for Staff #4 has a documented hire date of 09/23/2021. The date on the sworn statement for Staff #4 is 09/30/2020.
Plan of Correction: Will ensure that all dates are correct when filling out new employee paperwork.
