Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on July 19, 2021 and concluded on July 20, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 156 children present, ranging in ages from 5 to 12, with 23 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-130-C
Description: Based on review of five children's records, one did not contain the required documentation.
Evidence
The record for child number one does not contain an immunization record..

Plan of Correction: Going forward we will ensure all immunization records are received before a child's first day.

Standard #: 22VAC40-185-510-J
Description: Based on review of medication procedures, medications are not being stored in a locked place using a safe locking method that prevents access by children.
Evidence
Medications are being kept in unlocked fanny packs that are worn by staff members.

Plan of Correction: We will ensure all medication is locked up.
