Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 08/02/2021 and concluded on 08/03/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 76 children, ages 5 years-14 years, and 13 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 4 children?s records, 4 staff records and 2 medications were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 22VAC40-185-130-C
Description: Based on record review and discussion with the staff, the facility failed to obtain documentation of additional immunizations once between each child?s fourth and sixth birthdays.

Evidence: The record for Child #2 had a documented birth date of 06/03/2011. The most recent immunization record for Child #2 was dated 08/12/2013; there was not documentation of additional immunizations occurring between the child?s fourth and sixth birthday.

Plan of Correction: Emailed parent during the inspection to request the information.

Standard #: 22VAC40-185-510-N
Description: Based on document review and discussion with the staff, the facility failed to ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence: The authorization for Medication #1 expired on 07/01/2021. The medication was on-site during the 08/03/2021 inspection and a renewed authorization has not been obtained.

Plan of Correction: Will contact the parent/guardian following the inspection to get a new form.
