Comments:
An unannounced monitoring inspection was conducted Thursday, March 21, 2019 from approximately 3:30 PM until 5:15 PM to determine compliance with religious exempt child day center requirements. During the inspection, there were 31 children being supervised by four staff workers with the correct child to staff ratios being followed. Children were observed singing and dancing, participating in devotions, eating snack and playing with toys and games. The facility's classrooms, hallways and bathrooms were inspected. The playground was not inspected due to inclement weather. Management stated transportation is not provided at this time. Five children's and five staff records were reviewed. Policies and procedures were discussed and reviewed. See attached notice for violations found during this inspection.

The provider must send documentation to the licensing inspector that the background checks have been requested no later than 10 days following this notification dated March 21, 2019.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within five calendar days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of five staff records and interview, it was determined that the religiously exempt child day center did not ensure that all staff had a sworn statement of affirmation prior to employment and a search of the central registry within 30 days of employment for two staff members. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date June 2018) did not contain the central registry findings. The record for Staff #4 (hire date January 2019) did not contain a sworn statement of affirmation. Staff #2 and Staff #4 were observed working during the inspection. Staff #1 verified that these documents were not located in the records and could not be located during the inspection.

Plan of Correction: Required documents attached for Staff #2 & #4. New procedure implemented that requires Daycare Coordinator to complete checklist of required documents for all new employees and verified by Daycare Director.

Standard #: 63.2(17)-1716-B-3
Description: Based on the review of five staff records and interview, the religious exempt child day center did not ensure there was at least one staff member present that had been trained to conduct daily simple health screenings of children. Evidence: During the inspection, the licensing inspector reviewed the five staff records. The records did not contain documentation of daily health screening training. Staff #1 stated the staff currently working did not currently have daily health screening training.

Plan of Correction: All staff members will be required to complete Daily Health Screening Training.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of five staff records, one staff member did not obtain satisfactory results of the fingerprint-based national criminal background check prior to being hired. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date June 2018) did not contain satisfactory results of the fingerprint-based national criminal background check. Results of a finger-print based national criminal record search are required to be obtained prior to being hired. Staff #2 was observed working during the inspection. Staff #1 verified the background check was not located in the record and could not be located during the inspection.

Plan of Correction: Staff #2 fingerprint Based National Background Criminal Record Search scheduled for 03/29/19. New procedure implemented as stated above.
