Comments:
Thank you for your time and assistance with the unannounced complaint inspection which was conducted on 03/25/2019 and 04/02/2019. At the time of arrival there were 52 children in the care of 10 staff members. The inspection was in response to a complaint that was received in the Valley Licensing Office on 03/20/2019. The complaint was regarding staffing and supervision, ratio, policy and procedures for arrival and departure of children, and an incident that occured in December 2018. Four children records, one staff member record were reviewed, four staff were interviewed, video footage of an incident that occurred in December 2018 was reviewed, and parent and employee handbook were reviewed. The information gathered during the investigation does support the allegation of ratios being maintained at all times, so the complaint is determined to be " valid." Description: Based on observation, the center failed to ensure that the ratio of one staff member to four children for ages zero to twenty-four months was maintained at all times. 1. In the Rainbow Room during naptime there were six children present with one staff member. The youngest child in the room was 12 months date of birth 03/14/2018. The required ratio for children ages zero to twenty four months is 1:4 at all times of the day. 2. Staff verified that there was only one staff member in the classroom during naptime, and the ages of the children in the classroom were between 12 months- 24 months.

Plan of Correction: Administration were unaware that ratio could not be doubled with children zero-twenty four months. Administration has changed staff shift hours to ensure that there are always two staff if there are over four children ages zero to twenty-four months in the classroom.
