63.2(17) License & Registration Procedures

An unannounced monitoring inspection was conducted on 4/4/19 from 10:00 am until 12:45 pm to review the religious exempt requirements. At the time of the inspection 18 children were present with three staff. The sample size consisted of three staffs' records and five children's records. Children and staff were observed during outdoor play, bathroom break, hand washing, cleaning tables before snack, snack, educational activities, transitions, and behavioral guidance. Description: Based on record review and interview, the center failed to ensure all staff completed a sworn disclosure statement prior to the first day of employment. Evidence: 1. The records of three staff were reviewed. The records did not contain documentation of a sworn statement being completed. 2. The administrator stated they have not completed sworn statements for the staff.

Plan of Correction: An email was sent out on 4/5/19 asking each employee to sign the sworn statement by 4/10/19. A signed sworn statement will be required by every new employee before they begin work at GCS. A checklist will be placed in each employee's file to assure all paperwork is completed and up to date.

Description: Based on interview, the center failed to ensure a procedure was established and implemented to ensure a daily simple health screening and exclusion of sick children by a person trained to perform such screenings is completed. Evidence: Per the administration during the school day a nurse is present and if teachers feel the children need a health screening they send their student to the nurse. The teachers are not trained in daily simple health screening. After school the nurse is not present and the staff have not been trained to conduct daily simple health screening.

Plan of Correction: Three of our staff members will be registered to take the "Health Screening for Child Care Professionals" course offered by VDSS. The Administrative Assistant at GCS will be coordinating these efforts to ensure compliance.

Description: Based on record review and interview, the center failed to ensure staff hired after 1/22/18 had a fingerprint-based criminal history check determination letter prior to the first day of employment, and that staff hired before 1/22/18 obtained a fingerprint-based criminal history check determination letter by 9/30/18.


1. The records for three staff were reviewed. The records did not contain documentation of fingerprint background checks being completed. Two staff started employment prior to 1/22/18 (staff 1 started 8/12 & staff 2 started 8/15).
2. Per the administrator staff 3 started employment 1/19 and the center does not have the results of a fingerprint background check.
3. The administrator stated all staff completed fingerprint background checks but they never received results. They were unaware they needed to follow-up. Upon further research, during the inspection, it was found the center completed the fingerprint background checks incorrectly and they would all need to be redone.

Plan of Correction: An email was sent on 4/5/19 instructing all employees to log into Fieldprint and schedule an appointment to be fingerprinted. They must schedule the appointment by 4/10/19. Every new employee and those employees who have been employed by GCS for 5 years will be required to be fingerprinted by Fieldprint before beginning or continuing work at GCS. The date of fingerprinting and results of the screening will be recorded and filed in each employee's file. A checklist and spreadsheet will be reviewed yearly.
