Comments:
An unannounced complaint inspection which was conducted on 04/04/2019 from 12:10 P.M.-2:15 P.M. The inspection was in response to a complaint that was received in the Valley Licensing Office on 04/04/2019. The complaint was regards to appropriate supervision of children. One child's record, two staff records, four staff were interviewed, video footage of an incident that occurred on 03/14/2019 was reviewed. The information gathered during the investigation does support the allegation, so the complaint is determined to be " valid." The exception to compliance are noted on the violation notice. Description: Based on staff interviews and review of classroom video footage dated 03/14/2019 and center documentation, the center failed to implement their policy regarding appropriate supervision of children. Evidence: 1. On March 14, 2019 there were seven children ranging from 16 months-24 months of age with Staff A and Staff B in Classroom A. At approximately 12:20 P.M. Staff A yanked a cot forward with Child A, age 20 months, on it in a crawling position. Child A fell forward injuring her mouth on the edge of the cot. 2. Staff B verified that Staff A yanked the cot while Child A was on the cot in a crawling position. Child A fell forward and injured her mouth on the edge of the cot. Staff B told Staff A "I think she has hurt her mouth". Child A had placed her hands up to her mouth. Staff B verified that the child was bleeding and tried to place ice on her mouth. 3. Center documentation stated, "Child A was told multiple times to remain laying down on her mat I went to move Child A's mat as I was doing so she continued to move as that happened she fell forward and bit her bottom lip." 4. The center's policy states that "staff must inform the administration of a child's injury, and the administration will call a parent and in the event of a serious injury, get IMMEDIATE help by notifying Administration." Staff A and Staff B did not notify administration of the injury. Administration was notified by the child's parent after 5:30 P.M. at pick-up. 5. Staff verified that they did not notify administration, and that a phone call was not made to the parents. The parents were made aware of the incident from the accident report that was left in Classroom A.

Plan of Correction: 1. Staff A is no longer employed at the center. Staff B was cited for not following center policy. 2. Cots were removed from all classrooms and replaced with rest mats. 3. All staff have been verbally reminded of and re-read center's policy on injuries. 4. All mats and cribs have been moved so as to be in view of classroom video cameras.
