22VAC40-665 Subsidy Program Vendor Requirements for Family Day Homes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 06/05/2021 and concluded on 06/11/2021. The vendor was contacted by telephone to initiate the inspection. There were two children present at eight points.
The information gathered during the inspection determined that there were violations with applicable subsidy regulations.
Standard #: 22VAC4-665-340-F Description: Based on interviews and observations, the vendor failed to ensure that all children under the age of 12 months has a crib for rest periods.
1. The vendor had two children under the age of 12 months that did not have a crib to use for rest periods.
Plan of Correction: The vendor will purchase two cribs and will make sure that in the future all children 12 months and younger will have a crib.
Standard #: 22VAC40-665-160-B Description: Based on interviews, the vendor failed to ensure that a written hard copy record of daily attendance was completed.
1. The vendor stated that a daily attendance record has not been completed.
Plan of Correction: The vendor asked for the model form to be sent via email. The model form will be used starting tomorrow.
