An unannounced monitoring inspection was conducted Friday, May 10, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 8:30 AM and concluded at approximately 10:10 AM. During the inspection, there were a total of 26 children (two to five years of age) being supervised by six staff members. Children were observed practicing their end of the year program, taking bathroom breaks and washing their hands and eating snack. An End of the Year carnival was also taking place where children played games, created crafts and sang for their families. All areas of the center were inspected including the classroom, hallways, bathroom and outdoor play area. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. See attached notice for a violation found during the inspection. Evidence:
During the inspection, the licensing inspector observed the record for Staff #2 contained a sworn statement dated 08/16/2013. Background checks, including sworn statements are required to be updated at least every five years.Plan of Correction: Corrected during the inspection.
