An unannounced inspection was conducted from 11:10 - 1245. The area used by the program and classes were observed. There were 53 children being supervised by 9 staff and 2 staff present to answer administrative questions. The program had various activities ongoing and medication procedures were also checked. Standard #: 22VAC40-191-40-C-2 Description: Based on record review of 12 staff files, it was determined the program had not obtained search of central registry for 8 staff. Evidence: 1. All 12 Staff records contained Finger Print checks and Sworn Disclosure statements but only 4 staff records contained current CPS checks. Plan of Correction: We will all have staff complete clearance form and obtain notarization- these forms will all be sent with cashier's check.
Standard #: 63.2(17)-1716-A-6 Description: Based on record review, the program's operations are described in a written statement, but is missing the health requirements for the staff and public liability insurance. Plan of Correction: We are adding this information to the parent's board.
