Discussed clearances and that future staff requirements if he or she has lived outside of the commonwealth in the last 5 years.
- Comments:

An unannounced inspection was conducted from 930- 1115. The classrooms and classes were observed and all staff were aware of special care procedures. The staff have First Aid and CPR and written disclosures were available in posted form. The allergies and MAT requirements were also reviewed and staff were aware of any health issues that may occur. The program's facility will be in a different space in the Church next year and parents have been made aware of these changes. Please call with questions 703-479-4679

Standard #: 22VAC40-191-40-C-2 Description: Based on record review of staff files, it was determined the program had not obtained a sworn disclosure statement and search of central registry for each current staff member. Evidence: 1. Staff files had Finger print and state police background checks but did not have the CPS checks and Sworn Disclosure statements. Plan of Correction: The Inspector gave us a blank form for each record. We will have all staff fill each form out and we will send the CPS checks for current staff.
