Technical Assistance:
1. Discussed importance of keeping cleaning supplies out of reach of children, and electrical outlets covered. The daily challenges of shared activity areas acknowledged. 2. Reviewed background check requirements and timelines.

Comments:
10:15am-12:30pm Conducted an unannounced religious exempt inspection. Observed 58 children + 10 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities, to include: art, blocks, sensory table activities, cleaning up toys, and creative play. Staff were actively engaged with the children and activities. The facility was found to be very clean, safeguarded appropriately, and well supplied with age appropriate toys and equipment for the children. The outdoor playground was in need of additional resilient surfacing materials. Policies, procedures, and records were reviewed. Areas of non-compliance include record keeping, and physical plant safety.

Violations:
Standard #: 63.2(17)-1716-A-6
Description: Based on records reviewed, it was determined that required information was not disclosed to parents, guardians and the general public in writing. Evidence: (1) Health requirements for staff. (2) Public liability insurance.

Plan of Correction: This information will be put in writing for the fall.

Standard #: 63.2(17)-1716-B-6
Description: Based on observations made and interview conducted, it was determined that the resilient surface on the outdoor playground has not been maintained in accordance with federal playground safety guidelines. Evidence: The pea gravel in fall zones was not maintained at a minimum depth of 9". Today the slide fall zones were down to a hard packed dirt surface. Staff reported that pea gravel is replenished once a year.

Plan of Correction: The playground surface will be maintained in accordance with safety guidelines.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on records reviewed, it was determined that 1 of 8 staff records reviewed did not include out-of-state child abuse and neglect searches (CPS). Evidence: Staff #5 - The sworn disclosure statement, completed on 7/5/18, indicated that the individual has lived in multiple states in the last 5 years however, a request for the out-of-state CPS checks has not been completed.

Plan of Correction: Moving forward, out-of-state CPS checks will be completed, when required.
