1. Reviewed background check requirements, and associated timelines. 2. Discussed the storage of hazardous items out of reach of children.

10:10-11:50am Conducted an unannounced religious exempt program inspection. Observed 42 children + 10 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities, to include: letter and number recognition, counting, music, art, name writing, blocks, circle time, and active outdoor play. Staff were actively involved with the children and activities. The facility was found to be very clean, and well supplied with toys and equipment for the children. Staff are trained in First Aid/CPR. Background checks, training, and safety violations found.

Standard #: 22VAC40-191-40-C-2 Description: Based on records reviewed, it was determined that 4 of 8 staff records did not include required background checks. Evidence: 1. Staff #1 - CPS check. 2. Staff #2 - CPS check. 3. Staff #3 - Sworn disclosure statement and CPS check. 4. Staff #7 - CPS check. Plan of Correction: Required background checks will be completed.
Standard #: 63.2(17)-1716-B-3 Description: Based on interview conducted, it was determined that staff have not been trained in daily health screening. Evidence: The Director stated that while they do complete daily health screening of children, no one has been trained. Plan of Correction: Staff will complete training.
Standard #: 63.2(17)-1716-B-6 Description: Based on observations made, it was determined that an area of the school was not free of injury hazards. Evidence: The resilient surfacing (wood mulch) on the playground was less than 6", and in fall zones was down to the dirt. Plan of Correction: Mulch will be maintained in accordance with federal safety guidelines.
Standard #: 63.2(17)-1720.1-A Description: Based on records reviewed, it was determined that 4 of 8 staff records did not include a CPS check in the past 5 years. Evidence: 1. Staff #4 - Previous CPS check was completed on 2/3/14. 2. Staff #5 - Previous CPS check was completed on 2/3/14. 3. Staff #6 - Previous CPS check was completed on 2/3/14. 4. Staff #8 - Previous CPS check was completed on 2/3/14. Plan of Correction: Required background checks will be completed.
