Comments:
An unannounced monitoring inspection was conducted on 5/16/19 from 10:30 am until 12:45 pm to review the religious exempt requirements. At the time of the inspection 22 children were present with four staff. The sample size consisted of five children's records and all six staff's records. Children and staff were observed during free indoor play, educational and artistic activities, hand washing, bathroom break, lunch, transitions, and behavioral guidance. Violations were found during this inspection and are documented on the violation notice. If you have questions or concerns contact the licensing office at (540) 292-5933 for further assistance.

Standard #: 22VAC40-191-40-D-2
Description: Based on record review and interview, the facility failed to ensure all staff completed a central registry record check (CPS) within 30 days of employment. Evidence: 1. The records of all six staff were reviewed. Staff 1's start date is documented as 11/18. Documentation of a completed CPS was not found in Staff 1's record. Staff 3's start date is documented as 8/18. Documentation of a completed CPS was not found in Staff 3's record. 2. The director stated she thought the fingerprint background checks took place of the CPS record checks.

Plan of Correction: The director will provide both staff with the paperwork to complete and have notarized today. The documents will be mailed as soon as they are returned by staff. In the future the director will have staff complete the paperwork upon application and will mail the documents upon hire.

Standard #: 63.2(17)-1716-B-4
Description: Based on interview, the facility failed to implement a procedure to ensure that a first aid certified person is present at the center whenever children are present. Evidence: 1. The director stated the policy is someone is always present that is certified in first aid. 2. Two staff provided a copy of their first aid certificate. Staff 3's first aid certificate expired 9/18 and staff 6's certificate expired 8/18. 3. The director was unable to find any other staff with current first aid.

Plan of Correction: The last day of school is June 7th. Due to the inability to find a first aid class prior to the end of the school year, all staff will be scheduled to attend a first aid class in August prior to the next school year. From now on the director will keep documentation of staff first aid certification for all staff to ensure the center always has a staff present with first aid training.
