Comments:
Licensing Inspector arrived at 10:15 and departed at 11:30 AM to conduct a subsidy health and safety inspection (SHSI) in conjunction with a religiously exempt child day center code compliance visit a subsidy health and safety inspection (SHSI). At the time of the inspection, there 26 children (ages infant through four years) in care with four staff. Children were observed watching television and participating in free play. The sample size was four staff records and five children's records.

Violations:
Standard #: 22VAC40-665-520-A
Description: Based on review of five children's records, it was determined that the facility did not ensure that a record was maintained for each enrolled child. Evidence: There was only one record available for child #1 and #2 (siblings).

Plan of Correction: Center management will ensure that there is a record available for each enrolled child and not maintain family records.

Standard #: 22VAC40-665-780-B
Description: Based on observation and interview, it was determined that the facility did not maintain a record of all emergency practice drills. Evidence: 1. Center management and staff stated that emergency drills take place. 2. There was no record available to review.

Plan of Correction: Center management will begin immediately to maintain records of all emergency drills.
