Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted June 13, 2019 and June 20, 2019. The inspection was in response to a self-reported incident that was received in the Valley Licensing Office on June 11, 2019. The violation, which was not related to the incident, was noted on the violation notice and the risk rating for the repeat violation was noted on the supplemental page. On June 13, 2019 at the time of the inspection, there were 32 children in six classrooms and in the care of six staff members. There were a total of 56 children enrolled in the program. Violations:
Standard #: 63.2(17)-1716-A-3-a
Description: Based on observation and verification from staff, the center failed to maintain the required ratio of one staff member to every four children for ages zero to 24 months in one classroom. Evidence: 1. On 06/13/2019 at approximately 3:30 p.m., Classroom C had one staff member caring for five children. The youngest child in the room was 16 months (Date of Birth 01/18/2018) and the oldest child in the room was 20 months (Date of Birth 09/23/2017). The required staff to child ratio for children age zero to 24 months is 1:4. 2. Staff #1 and Staff #2 both confirmed that Classroom C was out of ratio based on the number and the ages of the children present.

Plan of Correction: On the day of violation the teacher responsible for ratio coverage had walked out during her lunch and did not return. Administration was covering another classroom and could not step in. We have been attempting to hire more staff for several months and will continue to do so. By the following day and in the future, the ratios have been and will be covered.
