Comments:
An unannounced monitoring inspection was conducted on 6/28/19 at Cornerstone Church of Augusta from 10:00 am until 12:30 pm to review the requirements for religious exempt facilities. At the time of the inspection 15 children were present with five staff. The sample size consisted of five children's records and five staff's records. Children and staff were observed during outdoor free play and water play, nap, transitions and behavioral management. Violations were found during this inspection and were documented on the violation notice. If you have questions or concerns contact the licensing inspector at (540) 292-5933 for further assistance.

Standard #: 22VAC40-191-40-D-2
Description: Based on record review and interview, the center failed to obtain a central registry record check (CPS) within 30 days of employment. Evidence: 1. Staff 1's first day of employment was 2/19/19 and staff 2's first day of employment was 12/13/18. Neither record contained documentation of CPS results. 2. The director stated she did not complete CPS searches for staff 1 and staff 2. She thought once fingerprints were required CPS searches were no longer required.

Plan of Correction: Documents for the CPS search was completed by both staff and put in the mail during the inspection. In the future all new staff will have a completed CPS search within 30 days after employment.

Description: Based on record review and interview, the center failed to establish and implement a procedure to ensure all staff are able to recognize the signs of child abuse and neglect. Evidence: 1. Five staff records were reviewed. The hire dates for the five staff are as follows: staff 1 2/19/19, staff 2 12/13/18, staff 3 9/14/14, staff 4 12/5/17 and staff 5 8/22/16. All five completed training in recognizing child abuse and neglect in 5/19 and 6/19. 2. The director stated prior to 5/19 the center did not have a policy for staff to be trained to recognize the signs of child abuse and neglect.

Plan of Correction: The current center policy is for all staff to receive training in recognizing the signs of child abuse and neglect prior to working with children.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the center failed to ensure staff hired after 1/22/18 had a fingerprint-based criminal history check determination letter prior to the first day of employment. Evidence: 1. Staff 1's first day of employment was 2/19/19 and fingerprint results are dated 3/22/19. Staff 2's first day of employment was 12/13/18 and fingerprint results are dated 1/10/19. 2. The director verified the dates. The director stated she was not aware fingerprints had to be completed prior to the first day of employment.

Plan of Correction: In the future staff will not start employment until fingerprint results are received.
