Technical Assistance:
We discussed the requirements for immunization reports and written parent notification. The risk assessments for violations was provided. Also, we talked about behavior management methods.

A complaint investigation was conducted during an interview on April 2, 2019 and an inspection on May 6, 2019 from 1:50 PM to 4:52 PM. After a thorough investigation involving observations of the facility and program, interviews with relevant persons, a review of pertinent documentation, a review of records for one child two staff, it was determined that the complaint regarding appropriate supervision was not valid. However, there were violations not reported, but found regarding a child's record and the required written notice to parents about the religious exemption. It is expected that the corrective action will be adhered to and compliance maintained with the Code of Virginia for religious exempt facilities. Thank you for your cooperation. Description: Based on a review of written information provided to parents, and interviews with administrative staff, the center failed to disclose in writing to the parents or guardians of the children in the center the fact that it is exempt from licensure. Evidence: A Statement of Code Compliance was completed by staff member 4 on June 1, 2018 and received at the Valley Licensing Office on June 5, 2019 with the beginning statement that Good News Ministries "...has disclosed in writing to the parents or guardians of children enrolled that the center is exempt from licensure...". The statement could not be located in the parent handbook. Staff member 1 and 4 indicated that the written information given to parents did not include a statement that the center is a religious exempt program and is not licensed as a child day center.

Plan of Correction: The director added a statement about religious exemption that was shared with parents in May and then clarified the information to meet the requirement for notification on June 7, 2019 and the updated statement was given to enrolled families. The parents of newly enrolling children will be provided this required written notice.

Description: Based on a review of a child's record, and interviews with administrative staff, the center did not ensure that all children in the center were in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases. Evidence: Child 1's enrollment date was written as December 3, 2018 as a three year old. The immunization report was completed on May 1, 2015 when the child was less than two months in age and was not a complete record for the child at the time of enrollment. Administrative staff indicated that the immunization report was from the period of attendance by child 1 during early infancy and the need for an up-to-date report was not noticed.

Plan of Correction: The director will be using checklists to document what was submitted by parents at the time of enrollment. Children will not be allowed to attend until all required information in the checklist is provided to the center. The director will view the documents to make sure all required information was received before enrollment of children.
