Comments:
An unannounced subsidy health and safety inspection was conducted on 9/5/19 from 10:40 am until 1:40 pm at Effort Baptist Church to review the subsidy health and safety requirements. At the time of the inspection 84 children were present with 7 staff. The sample size consisted of seven staff's records and seven children's records. Children and staff were observed during free indoor and outdoor play, bathroom break, hand washing, story time, nap, departure, transitions and behavioral management. There were violations found during this inspection and they are documented on the violation notice. Violations:
Standard #: 22VAC40-665-520-B-8
Description: Based on record review and interview, the vendor failed to ensure a written authorization for emergency medical care is obtained for all children if an emergency occurs and the parent cannot be located immediately unless the parent presents a written objection for the provision of medical treatment on religious or other grounds.

Evidence:

1. Seven children's records were reviewed. None of the children's records contained a written authorization for emergency medical care.
2. The director stated they do not obtain a written authorization for emergency medical care.

Plan of Correction: A written authorization for emergency medical care will be developed and sent to all parents of children in care. In the future the authorization will be part of the enrollment packet.

Standard #: 22VAC40-665-560-B
Description: Based on record review and interview, the vendor failed to obtain additional immunizations once every six months for children less than two years of age.

Evidence:

1. The record for child 4, date of birth 3/14/17, contains an immunization record dated 3/5/18.
2. The director verified the 3/5/18 immunization record is the most current record they have.

Plan of Correction: An updated immunization record will be obtained for child 4. In the future the administration will ensure all immunization records are up-to-date.

Standard #: 22VAC40-665-580-B
Description: Based on record review and interview, the vendor failed to ensure all new staff complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment.

Evidence:

1. The records of seven staff were reviewed. Staff 3's start date was 1/19/19. There was no documentation of staff 3 completing the preservice training. Staff 5's start date was 5/28/19. There was no documentation of staff 5 completing the preservice training.
2. The director verified staff 3 and staff 5 have not completed the preservice training.

Plan of Correction: Staff 3 and staff 5 will be advised to complete the Virginia Preservice Training for Child Care Staff by the end of September. In the future the administration will ensure that all new staff complete the training within 90 days of employment.
