Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 4 children (5points) directly supervised by 1 caregiver/ vendor. Ratio and supervision were in compliance. Children were observed: creating beads, eating snack and getting ready to go outside. Areas of non-compliance are identified in this report. The exit interview was conducted with the vendor/provider. Violations:
Standard #: VENDHOM-000-005-B
Description: Based upon record review, vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was no written record of attendance for the 4 children in care at the time of the inspection. 1 child had been in care all day and 3 had just arrived at 3:30 pm when they got home from school.

Plan of Correction: The vendor created attendance at the time of the inspection.

Standard #: VENDHOM-000-006-B
Description: Based upon record review, required child information was found to be missing or incomplete. Evidence: Child A is missing parent work address and phone number, emergency contact address and phone number, medical insurance information and emergency authorization for medical treatment signed by parent. Child B is missing parent work address and phone number, emergency contact address and phone number, medical insurance information and emergency authorization for medical treatment signed by parent. Child C is missing parent work address and phone number, medical insurance information and emergency authorization for medical treatment signed by parent. Child D s missing parent work address and phone number, medical insurance information and emergency authorization for medical treatment signed by parent.

Plan of Correction: Per vendor's grand-daughter, vendor will have parents sign new forms that were printed out during the inspection.

Standard #: VENDHOM-000-011-A
Description: Based upon interview, the vendor, and any caregivers who are left alone with children, was not able to speak, read, and write in English sufficient to understand the regulations and communicate with the department, and local department personnel. Evidence: The vendor was unable to communicate with inspector at the time of the inspection. Vendor's grand-daughter translated questions from inspector to vendor regarding child information.

Plan of Correction: Per grand-daughter, the vendor will contact county to see who can help.

Standard #: VENDHOM-000-026-E
Description: Based upon observation, the diapering surface was no longer non-absorbent as required. Evidence: The blue diaper changing mat had rips going down the sides of the mat.

Plan of Correction: Per vendor's grand- daughter they will buy a new mat.
