Technical Assistance:

1. Reviewed all background check requirements, and associated timelines and processes.

Comments:

10:55am-2:00pm Religious exemption inspection conducted of the preschool program. Observed 96 children + 20 direct-care staff. Ratios and supervision were in compliance. Children were observed in a variety of activities, to include: carving a pumpkin, art, puzzles, circle-time, indoor-free play, and outdoor active play, and lunch. Staff were actively engaged with the children and activities. An after-school program is offered for both preschool and school-age children. The facility was found to be clean, and well supplied with age appropriate supplies and equipment for the children. Safety and administrative non-compliances found.

Standard #:	22VAC40-665-620-A
Description:	Based on observations made, it was determined that hazardous substances were not kept in a locked place that prevents access by children. Evidence: Household cleaning wipe products were found to be accessible to children in several classrooms and bathrooms.
Plan of Correction:	Staff will be reminded to store all cleaning supplies appropriately when not in-use.
Standard #:	63.2(17)-1716-A
Description:	Based on observations made, it was determined that a written disclosure to parents/guardians of the children in the center about required information was not posted in the facility. Evidence: Licensure exemption and staff qualifications were not posted in a visible location on the premises.
Plan of Correction:	This information will be posted on the bulletin board outside of the preschool office.

Description:	Based on records/documents reviewed, it was determined that a written disclosure of required information has not been made to parents/guardians and the general public. Evidence: Information about the enrollment capacity and public liability insurance have not been disclosed in writing.
Plan of Correction:	This information will be put in writing.
Standard #:	63.2(17)-1720.1-B-3
Description:	Based on records reviewed and interview conducted, it was determined that 1 of 11 staff records did not include the results of out-of-state child abuse and neglect searches. Evidence: Staff #10 - Out-of-state searches were sent to two required states in November 2018 however, the results of these searches were not on-file. Staff stated they were not sure where the results were, or if they were received.
Plan of Correction:	Staff will follow-up with each state to determine if the submitted search was completed, and if so, obtain the results and place them in the staff record. If not, new search requests will be submitted within 10-days. Copies of the submitted report and/or results of the searches will be sent to the inspector within 10-days.
