Standard #: 63.2(17)-1720.1-A Description: Based on the review of a sampling of the 8 staff files, it was determined that the center failed to get updated sworn disclosure statements for some of the staff.
Evidence: 5 out of 8 staff do not have sworn disclosure statements that have been done in the last 5 years. The staffs sworn disclosure statements were done between 2004 and 2012. Staff missing updated sworn disclosures: staff A, C,D,G and H.
