63.2(17) License & Registration Procedures

Technical Assistance:
1. Discussed supervision practices, and the importance of striving to maintain sight and sound of children at all times, with the exception of the older children when toileting.
2. Staff bags/purses should be considered to have hazardous products/items and kept out of reach of children.
3. Reviewed background check requirements.
4. Record retention policy discussed.

Comments:
10:25am-1:45pm Unannounced religious exempt inspection conducted. Observed 87 children + 17 direct-care staff. Children were observed in a variety of activities, to include: sensory exploration, dramatic play, writing letters/numbers, art, library time, painting, and active outdoor play.The facility was found to be clean, well maintained, and well supplied with age appropriate toys and equipment for the children. Staff are currently trained in First Aid, CPR, E-MAT, child abuse + neglect. Health, safety, and administrative non-compliances found.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on interview conducted, it was determined that 1 of 8 staff records requested did not include a signed sworn disclosure statement (SDS) prior to the first day of employment. Evidence: Staff #8 was observed to be working today, and has not completed a SDS. It was reported to be Staff #8's first day of work.

Plan of Correction: Staff #8 will sign the SDS prior to returning to work. Moving forward, this will be required prior to the first day of work.

Standard #: 63.2(17)-1716-A
Description: Based on records reviewed,it was determined that required information has not been disclosed in writing to the general public. Evidence: Information about physical facilities, enrollment capacity, food services, health requirements for staff, and public liability insurance.

Plan of Correction: This information will be put in writing in a format available to the general public. Completed during inspection.

Standard #: 63.2(17)-1716-A-3-c
Description: Based on observations made, it was determined that staff-to-child ratios exceeded 1:8 in one classroom today. Evidence: For a period of minutes, the ratios in a 2 year-old classroom were 1:9.

Plan of Correction: Staff will not leave classrooms our of ratio in the future, and will seek assistance when needed.

Standard #: 63.2(17)-1716-A-5-d
Description: Based on records reviewed, it was determined that transportation procedures did not require drivers to have a valid driver's license. Evidence: The contract, signed by all parents, did not include this requirement.

Plan of Correction: This requirement will be added to the existing contract for the 2020-2021 school year. Before spring fieldtrips, drivers will be asked to sign a new contract.

Standard #: 63.2(17)-1716-B-5
Description: Based on observations made, it was determined obvious hazards were accessible to children. Evidence: Product labeled as "keep out of reach of children" were accessible to children in 4 of 8 classrooms. Products included household cleaning wipes, witch hazel, and a "chill spray."

Plan of Correction: Products labeled "keep out of reach of children" will be stored in such manner.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on interview conducted, it was determined that 1 of 8 staff records requested did not include a criminal record check (CRC) prior to the first day of employment. Evidence: StStaff #8 was observed to be working today, and has not completed a CRC. It was reported to be Staff #8's first day of work.

Plan of Correction: Staff #8 will complete a CRC, and the results will be on-file, prior to returning to work. Moving forward, this will be required prior to the first day of work.
