An unannounced inspection was conducted from 8:50- 10:05. The program uses two floors and has a multi activity room. The program had children ages 3 years to 5 years and ratios were met with 57 children with 11 staff actively supervising. The program does not have transportation and does not leave the premises for field trips. Please call with questions 703-479-4679

Standard #: 22VAC40-191-40-D-2 Description: Based on record review and interview, it was determined the Program had not maintained a Central Registry Search for Staff #2.
Evidence:
1. Staff records reviewed and Staff #2 had been en employee for over a year and the file did not include a Central registry Search.
2. The Director stated Staff was a sub and that it could be in the Sub files but was unable to locate.Plan of Correction: I will continue to look for it but will also have the staff fill out today and send out at the end of the week if it is not located.
