Technical Assistance:
Discussed Background checks for future staff- if staff have lived outside of commonwealth in last 5 years, program needs to submit a Central registry search for that person in the state she or he resided. Comments:
An unannounced inspection was conducted from 8:550 10:15. The Program had 7 classes operating and ratios were in compliance and as follows: Toddler Class 6 to 2, Two's Group 5 to 2, Three's Group I 11 to 2, Three's Group II 13 to 2, Four's Group I 11 to 2, Four's Group II 12 to 2, and PreK Group 155 to 2. Staff were aware of procedures and observations included art activity, and inside play. Records were also reviewed and found to be in compliance. Description: Based on observation and record review, it was determined the Program did not describe in a written statement provided to the parents or guardians of the children in the center and made available to general public information about the physical facilities, food services provided, health requirements of the staff, and information about public liability insurance.
Evidence:
1. The Office had the Capacity posted in view. The board and the parent's handbook did not describe what part of the church was used by the program, health requirement that staff must obtain medical clearance each year, and information about public liability insurance.
2. The parent handbook stated what the food services were but it was not posted in a public area.

Plan of Correction: We will make corrections.
