Comments:
The licensing inspector for Hazel River Assembly of God conducted an unannounced complaint inspection on 12/3/19 from 7:20 am until 8:15 am and and 12/18/19 from 8:09 am until 8:30 am in response to a complaint that was received by the licensing office. Interviews were conducted with staff and observations were made relating to the allegation of not maintaining correct ratios and supervision. The information gathered during the investigation supports the allegations, so the complaint is determined to be "valid." Description: Based on observation and interview, the center failed to employ supervisory personnel according to the following ratio of staff to children: 1 staff to 5 children for ages 16 months to 24 months; 1 staff to 8 children for ages 24 months to 36 months; 1 staff to 10 children for ages 36 months to 5 years; 1 staff to 20 children for ages five years to nine years; and 1 staff to 25 children for ages nine years to 12 years.

Evidence:

1. A parent stated on 11/14/19 at 7:00 am there was at least herself with her two children (ages 19 months and and three), two preschool age siblings, another toddler and another preschooler waited at the door to be let in since they do not open until 7:00 am. Sometimes there are more. The only staff that was present was the assistant director. The children walked back to the gym on their own. The parent stayed in her car to wait to see if any other staff showed up. Another staff member showed up about 10 minutes later.
2. The director stated on 11/14/19 the staff that would normally be at the center when they open were caught in traffic due to a car accident. The second staff arrived at 7:12 am and the third arrived at 7:20 am. The director also stated as children arrive each morning they go into the cafeteria which is beside the entrance.
3. On 12/3/19 at 7:20 am the licensing inspector (LI) arrived at the center.
? 7:20 am - 21 children (ages two to 10) with two staff.
? 7:32 am - 28 children (two to 10) with two staff when Staff 3 arrived and took nine children to the bus, which left 19 children (two to four) with two staff. Staff 3 did not return to the cafeteria.
? 7:38 am - Staff 4 arrived and 27 children were present at this time in the cafeteria with Staff 1 and 2. Staff 4 then took five two-year old children which left 22 children with two staff in the cafeteria ages three and four.
? 7:43 am - eight children, age four, left with staff 5 when she arrived. Children continued to arrive but to go to their individual classes except the threes stayed in the cafeteria.
4. On 12/18/19 at 8:10 am one staff member was present with 13 children ages four and five.

Plan of Correction: 1. The door will continue to stay locked and closed until 7:00 am when facility opens and staff is contracted to arrive.
2. Two additional staff members have been hired to arrive at 7:00 am. One staff member will be assigned to ages two to three (24 to 36 months) and another for ages four and five (12/23/19).
3. Staff contracted to arrive after 7:00 am will be "on call" one day a week in case traffic incidents, sickness, etc. Administration will oversee and communicate to ensure no lapse (12/20/19). Additional staff member hired to arrive at 7:00 am (12/23/19).

Description: Based on observation, the center failed to ensure at least one staff member was present for every 10 children from ages 36 months to five years.

Evidence:

1. On 12/3/19 at 7:38 am 22 children ages three through four years were in the cafeteria with two staff.
2. On 12/3/19 at 8:00 am 15 children age four were with one staff in the pre-k classroom.

Plan of Correction: Two additional staff members have been hired to arrive at 7:00 am. One for each age group.
