Discussed the new ratio requirements for Religiously Exempt Child Day Centers that were effective 7/1/2019.

An unannounced compliance monitoring inspection was conducted on September 18, 2019 from 12:00 p.m. to 2:30 p.m. There were no children in care at the time of the inspection. Center documents, five children's records, and three staff records were reviewed.

Evidence: Staff #1, rehired 1/15/19, did not have documentation of sworn statement in the record. Plan of Correction: Form completed on 09/19/19
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records, it was determined that the center did not obtain documentation of national fingerprint background check results before the first day of employment. Evidence: Staff #1, rehired 1/15/19, did not have documentation of fingerprint check in the record. Plan of Correction: : Background completed on 09/30/19
