Comments:
Licensing Inspector arrived at 9:30 and departed at 10:45 AM to conduct a religiously exempt child day center code compliance inspection. At the time of the inspection, there were 74 preschool age children in care with 11 staff. Children were observed participating in table activities. The sample size was 29 staff records.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of 29 staff records, it was determined that the facility did not ensure that all staff had a completed search of the central registry on file within 30 days of employment.

Evidence:
The following staff, all employed over 30 days, did not have a completed search of the central registry on file: staff #14, staff #15, staff #17, staff #19, staff #20, staff #24, staff #25, staff #26, staff #27, and staff #28.

Plan of Correction: Facility will resubmit central registry checks and when results are received will place them in the records.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of 29 staff records, it was determined that the facility did not ensure that all staff submitted to fingerprinting as required prior to employment.

Evidence:
The following staff did not have completed fingerprints on file: staff #8 and staff #24/

Plan of Correction: Facility will either locate fingerprints or have staff fingerprinted again and then place the results in the records.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of 29 records, it was determined that the facility did not ensure that if a staff member had lived outside of the state of Virginia within the past five years, they obtained a completed search of the central registry for those states.

Evidence:
The sworn statement for staff #23 indicated that he had lived in North Carolina in the past five years. There was no completed search of the central registry for North Carolina on file.

Plan of Correction: Facility will have a search of the central registry for the state of North Carolina and place results in the record.
