Comments:
An unannounced monitoring inspection was conducted on 1/27/20 from 10:15 am until 1:45 pm at Effort Baptist Church to review the subsidy health and safety requirements. At the time of the inspection 58 children were in care with 12 staff. The sample size consisted of five children's records and five staff's records. Children and staff were observed during free indoor and outdoor play, educational and artistic activities, story time, library, diapering, bathroom break, hand washing, lunch, departure, transitions and behavioral guidance. Violations:
Standard #: 22VAC40-665-530-2-d
Description: Based on record review and interview, the vendor failed to obtain results of a Child Protective Service Central Registry check within 30 days of the individual's beginning date of employment.

Evidence:

1. The record of staff 4 did not contain documentation of a completed central registry.
2. The director stated staff 4 was rehired 8/27/19 after being gone for a couple of years and they missed redoing the central registry check.

Plan of Correction: A new central registry check will be completed and mailed.

Standard #: 22VAC40-665-580-B
Description: Based on record review and interview, the vendor failed to ensure all staff completed Virginia Preservice Training for Child Care Staff within the first 90 days of employment.

Evidence:

1. The records of staff 2 (start date 8/2/19), staff 3 (start date 8/10/19), staff 4 (start date 8/27/19) and staff 5 (start date 10/10/19) did not contain documentation of completing the training.
2. The director verified the staff had not completed the training.

Plan of Correction: Each staff will complete the training. In the future the director will ensure all new staff complete the training within the first 90 days of employment.

Standard #: 22VAC40-665-610-C-2
Description: Based on observation and interview, the vendor failed to ensure protective covers that are of a size that cannot be swallowed by a child cover all electrical outlets.

Evidence:

1. Two surge protectors on a table plugged into a wall under the TV in the library did not have outlet covers.
2. The center's policy is for all outlets not being used to have outlet covers or child proof covers.

Plan of Correction: The center has ordered two child proof surge protectors to replace the two surge protectors.
