During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Model forms will be emailed to the provider along with the training link for the 3 hour health and safety update training.

A subsidy health and safety inspection (SHSI) was initiated and completed on 06/25/2025. The inspection started at 10a.m. and concluded at 11:30a.m.. There were no children present and 1 caregiver the point total was 0. The inspector reviewed 2 children?s records and 1 caregiver and 1 household member records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-190-B Description: Each child's record shall include address for each custodial parent's place of
employment, proof of identity, emergency medical treatment authorization and the provider's medication policy.
Child #1 and #2's information in their records did not include addresses for parent's employment and authorization for emergency medical treatment.
Child #1's record was did not include proof of the child's identity.Plan of Correction: I have requested the birth certificate from this parent. I will call the specialist for the county to assist. Once I receive the forms for the child's records I will review with the parent, sign and file in records.
Standard #: 8VAC20-790-250-F Description: Repeat
Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.
The provider did not obtain 16 hours of annual training and did not complete the annual health and safety update course.Plan of Correction: Contact was made with the regional navigator who will assist with accessing the training.
