Comments:
An unannounced monitoring inspection was conducted on 3/3/2020 from 10:40 am until 12:30 pm at Beautiful Gate Ministry Inc./The Starting Gate Preschool to review the religious exempt requirements. At the time of the inspection 52 children were in care with nine staff. The sample size consisted of five children's records and five staff's records. Children and staff were observed during indoor and outdoor free play, educational activities, bathroom break, hand washing, transition and behavioral guidance. Violations from the previous inspection were reviewed for current compliance. No repeat violations were found. Violations found during this inspection are documented on the violation notice. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review and interview, the center failed to have staff sign a sworn disclosure statement prior to the first day of employment and have a completed central registry record check by the end of the 30th day of employment.

Evidence:

1. The records for five staff were reviewed.
Staff 1's start date per the administrator was 3/18/19. The central registry in the record is dated 12/18/19.
Staff 3's start date per the administrator was 9/2/19. The record for staff 3 did not contain documentation of a central registry background check.
2. The Reverend stated the previous administrator left on medical leave and the administrative work got behind. They are working on getting things organized and caught up.

Plan of Correction: All the records are being audited and systems have been put into place with the new administrator. Staff 3 will complete the paperwork for a central registry background check and it will be mailed within five days. The administrator will send an email to the licensing inspector when the paperwork is mailed to advise.

Standard #: 63.2(17)-1716-A
Description: Based on observations and interview, the center failed to post the fact that the center is exempt from licensure in a visible location on the premises.
Evidence:
1. A tour of the facility was conducted. No posting of the center's exemption status was found in either building.
2. The Reverend stated the exemption letter was posted at the door of the Little Gate building but is no longer posted. There was never an exemption posted in the Middle Gate building.

Plan of Correction: A copy of the exemption letter will be posted at the Little Gate building and the Middle Gate building.
