Comments:
An unannounced monitoring inspection was conducted at the center on 3/9/20 from 3:30 pm until 4:30 pm at Commonwealth Christian Community to review the religious exempt requirements and the staff records were reviewed on 3/20/20. At the time of the inspection 13 children were present with two staff. The sample size consisted of five children's records and four staff's records. Children and staff were observed during free outdoor play, hand washing, snack, pick-up, transitions and behavior guidance. Violations were found during this inspection and are documented on the violations notice. If you have questions or concerns contact the licensing inspector at (540) 848-4123 for assistance.

Standard #: 63.2(17)-1716-B-5
Description: Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining cushioning material under playground equipment.

Evidence:

1. The two swings under the tree have little to no resilient surfacing.
2. The director stated they are getting some mulch delivered this month.

Plan of Correction: In the future the center will ensure resilient surfacing is maintained.

Description: Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters that are dated no more than 90 days prior to the date of employment.

Evidence:

1. The administrator stated the start date for staff 4 was 7/1/19. The record for staff 4 was reviewed. The fingerprint-based criminal history check determination letter is dated 10/11/18.
2. The administrator stated staff 4 originally was going to start work in October of 2018 but changed her mind and did not start until July of 2019.

Plan of Correction: Staff 4 will redo fingerprints when fieldprint reopens. In the future potential staff will be required to complete fingerprints when they are ready to start work.
